Care Coordination
Care Coordination is guided by the needs of the individual that she is working with. It is influenced by the initial assessment that is undertaken over 1-3 sessions to determine the most appropriate service supports that can be offered based on Individual/ Familial and Support Network needs Plus, the capacity within the organisation at the time of the referral.
Care coordination can include:
Monitoring of responses to medication prescribed by your GP or other medical professionals.
Providing psychotherapy.
Undertaking regular assessments of an individual’s physical and mental health status.
Support to attend collaborative appointments with health care specialists.
Referrals via a GP for specialist health care supports.
Provide specialist health assessments to increase supports of others in the individuals life, to improve their quality and accessibility to their life as they want to live it.
Increase individual/familial and support network capacity by decreasing the many competing demands experienced by all.
