Care Coordination

Care Coordination is guided by the needs of the individual that she is working with. It is influenced by the initial assessment that is undertaken over 1-3 sessions to determine the most appropriate service supports that can be offered based on Individual/ Familial and Support Network needs Plus, the capacity within the organisation at the time of the referral.

Care coordination can include:

  • Monitoring of responses to medication prescribed by your GP or other medical professionals.

  • Providing psychotherapy.

  • Undertaking regular assessments of an individual’s physical and mental health status.

  • Support to attend collaborative appointments with health care specialists.

  • Referrals via a GP for specialist health care supports.

  • Provide specialist health assessments to increase supports of others in the individuals life, to improve their quality and accessibility to their life as they want to live it.

  • Increase individual/familial and support network capacity by decreasing the many competing demands experienced by all.
